  • HKID (Original)
  • Passport (Original & Copy)
  • Consulate letter (Original & Copy)
  • Immunization Record (Original & Copy)
  • Five recent passport-size photographs with white background
  • HK & USA Address and Email address (Copy)
  • Birth Certificate (Original & Copy)
  • Old chest x-ray film and report (If applicable)
  • Tuberculosis treatment report (If applicable)
  • Prescription eyeglasses / hearing aids (If applicable)
  • Names & doses of all long term medicine taken (If applicable)
  • COVID-19 Vaccination Record (Copy)

If applicant has not been vaccinated before the examination, please submit the COVID-19 Vaccination Record (Copy) to the clinic after receiving at least one dose of vaccination.

Terms & Conditions
  • Quality HealthCare will contact the customer within 5 working days after successful payment. Customers can also call physical check-up hotline at 8200 8825 for booking.
  • The physical check-up plan is available at designated Quality HealthCare Physical Check-up Centre.
  • Customers must present their identity cards and order confirmation letter (printed or e-version) on the appointment day.
  • Physical check-up plan is applicable to people aged 15 or above only.
  • The above check-up price does not include the price of additional tests and vaccination.
  • Physical check-up plan is not applicable on Sundays and Public Holidays.
  • The redemption will be valid for 6 months starting from the date of payment.
  • Neither amendment nor cancellation is allowed once the payment has been confirmed. Also, it is not transferable nor refundable.
  • All tests are not for the purpose of medical diagnosis or treatment.
  • Applicant aged below 18 must be accompanied by Parent(s).
