疫苗注射程序及注意事項 Vaccination Procedures & Remarks:

• 參加者請於疫苗注射期內注射(詳情請參閱注射信上日期)。Participants are required to take the vaccination during the vaccination period(Please refer to redemption letter for validity).
• 參加者必須透過卓健手機App預約流感疫苗接種服務 ◦Participants are required to make an appointment for flu vaccination services via QHMS mobile app.
• 接受疫苗注射前，必須出示本疫苗注射信，香港身份證/護照及填妥之疫苗注射同意書以供診所職員核實。The Redemption Letter, valid HKID card/passport must be presented to the clinic for verification & the consent form must be filled out at the clinic before vaccination.
• 年齡18歲以下之參加者(以注射日當天為準)必須由家長/監護人陪同，並代表他們在診所内簽署同意書。 Participants aged below 18 (age as at the date of vaccination) MUST be accompanied by their parents/guardians, who will sign on the consent form on behalf of the participants at the clinic.
• 如閣下有以下任何一種情況，請在接受疫苗注射前諮詢閣下醫生之專業意見：If you have any of the following conditions, you are recommended to seek medical advice before receiving vaccination:
  • 患有急性 , 慢性 或嚴重之疾病或其他疾病（不論發燒與否）例如上呼吸道感染、感冒 suffering from any acute, chronic or severe illness or any illness (with or without fever) e.g. upper respiratory tract infection, common cold
  • 對任何疫苗成分如蛋白、雞肉、硫酸慶大霉素（抗生素）、新霉素、甲醛（防腐劑）、凝血素等過敏。Allergic to any vaccine components, including egg white, chicken protein, gentamicin sulphate, neomycin, formaldehyde (preservative), haemagglutinin, etc.
  • 過往曾對藥物或疫苗過敏或嚴重反應 allergy or serious reaction to drug or vaccine in the past
  • 血液凝固毛病(例如血小板減少症、凝血障礙) blood disorder e.g. thrombocytopenia, coagulation disorder
  • 免疫系統受到抑制 immunocompromised
