Product Details:

  • Medical Certificate Renewal Services by Approved Medical Examiner (AME) appointed by The Hong Kong Civil Aviation Department
    • - Class 1 (CPL, MPL, ATPL, PPL/IR)
    • - Class 2 (PPL/ student pilot license)
    • Class 3 (Air Traffic Controller’s License)
  • Investigations (Blood test and imaging)
  • Specialists Referral
  • Fitness to Fly Evaluation

 

 Please bring along the following on the day of your visit:

  • Photo Identification (Hong Kong Identity Card/ Passport)
  • Latest and previous Medical Certificate DCA152
  • Current and previous Operator License
  • Previous Medical Report Forms DCA153, if available
  • Relevant Investigation Reports/ Specialist Evaluations, as required
  • Corrective Glasses/ Contact Lenses witn container, as required
  • Medications currently taken

 

Designated Quality HealthCare Physical Check-up Centre:

  1. Quality HealthCare will contact the customer in around 5 working days after the date of payment for appointment arrangement. Customers can also use e-Booking service on the Quality HealthCare Mobile App / WhatsApp us at 92208388 / call our physical check-up hotline at 8102 8068 for booking.
  2. This physical check-up plan is only available at designated Quality HealthCare Physical Check-up Centre(s) as shown above.
  3. Customers must present their identity cards and order confirmation letter (printed or e-version) on the appointment day for verification.
  4. The above check-up price does not include the price of additional tests.
  5. This physical check-up plan is not applicable on Sundays and Public Holidays.
  6. This physical check-up plan will be valid for 6 months starting from the date of payment. Customer must redeem and complete the relevant check-up within the 6 months’ period.
  7. No amendment to the physical check-up plan purchased is allowed once payment is confirmed. Any fees paid will not be transferable and will not be refundable.
  8. In case of disputes, the decision of Quality HealthCare Medical Services Limited shall be final.
